Patient non-adherence to prescribed treatment regimens remains one of the most pervasive and costly challenges in modern healthcare, contributing to suboptimal clinical outcomes, increased hospitalization rates, and significant economic burden across diverse therapeutic domains. Traditional adherence interventions, which predominantly rely on educational strategies and reminder systems, have demonstrated limited long-term efficacy, underscoring the need for more sophisticated, theory-driven approaches that address the cognitive, emotional, and behavioral determinants of health behavior change. This review research paper presents a comprehensive meta-analysis of past work examining the integration of cognitive science frameworks with evidence-based behavioral coaching methodologies to enhance patient engagement and improve treatment adherence. Drawing upon a systematic synthesis of thirty peer-reviewed studies spanning clinical psychology, behavioral medicine, cognitive neuroscience, and health coaching literature from 2010 to 2025, this analysis evaluates the theoretical foundations, intervention designs, outcome measures, and effect sizes associated with integrated cognitive-behavioral coaching models. Key cognitive science constructs including attentional control, working memory capacity, cognitive load management, self-regulatory processes, and decision-making heuristics are examined in relation to behavioral coaching techniques such as motivational interviewing, habit formation protocols, goal-setting frameworks, and feedback loops. The meta-analysis reveals that integrated interventions produce significantly greater adherence improvements (mean standardized effect size d = 0.89; 95% CI: 0.72–1.06) compared to single-modality approaches, with sustained benefits observed at six- and twelve-month follow-up intervals. Furthermore, this review identifies critical gaps in current research, including insufficient longitudinal tracking, variable measurement of cognitive engagement, and limited exploration of digital coaching platforms. The findings support the development of standardized, scalable integrative frameworks that combine cognitive assessment with tailored behavioral coaching to maximize patient engagement and optimize treatment adherence in clinical practice. |
